I wish to inform you that I will be closing my medical practice on December 31, 2025. Unfortunately, I have been unable to find a replacement for my practice.

I recognize that changing physicians may be stressful. I strongly encourage you to seek continuity of care by contacting Health Care Connect at 1-800-445-1822 or visiting ontario.ca/healthcareconnect for information on other physicians in the area.

Please note that programs and services offered from the OSFHT will no longer be available to you as of December 31, 2025.

Regarding Ongoing Medical Needs:

Outstanding Test Results:
After December 31, 2025, if you have any outstanding laboratory results, you can obtain them from the testing facility with written authorization, or you can view them online and discuss with your new physician or you can review them with the physician at the ER.

 Prescription Renewals:
Please ensure you obtain any necessary prescription renewals before your final visit. Unless it is a narcotic or controlled medication, your pharmacist is authorized to extend your prescriptions.

 Urgent/Emergent Care:
I will continue to provide urgent or emergent care until the practice closure date, December 31, 2025.

 Regarding Medical Records:
Since your medical records are confidential, a copy can only be transferred to another doctor or released to you with your written permission through an authorization for the release of medical records form.

 After December 31st, all my medical records will be transferred and stored at:

They can provide a copy of your records for a fee. (Unfortunately, it is not an OHIP insured service).

Please do not delay in finding a new family doctor. I encourage you to contact the resources mentioned above to begin your search for a new physician.
