Diabetes Care

The OSFHT Diabetes Program provides patients Living with type-2 diabetes the education and tools needed to manage their diabetes.  The program is run by Certified Diabetes Educators (Dietitans and Nurses) in collaboration with your Family Physician.

The program includes education and support with:

  • Nutrition and meal planning
  • Goal setting
  • Medications
  • Foot care
  • Self-monitoring blood glucose
  • Preventing complicationsof Diabetes

You need a referral from a physician or nurse practitioner at the OSFHT to enter the diabetes program.

Healthy Living with Type 2 Diabetes (Part #1) The Basics

What: Part 1 of a 2 part series on Type 2 Diabetes and its management.

Topics Covered:

  • What is Type 2 Diabetes
  • Healthy eating
  • Physical Activity
  • Medications
  • Blood glucose monitoring

Who:

  • For Patients newly diagnosed with Type 2 diabetes
  • For those already living with Type 2 diabetes who need an update.

When: A variety of options available each month. 

Length: Please note both classes in the series are 2.5 hours

Healthy Living with Type 2 Diabetes (Part #2) Carbohydrate Counting and More​

Carbohydrate Counting, also called carb counting, is helpful for people trying to manage blood sugars.

While many people with diabetes limit sugar, most can achieve better control by looking at Carbohydrates, which break down into sugar in our body.

This class will help you understand foods that contain carbohydrates, how they affect your blood sugar, and how to plan balanced meals for better diabetes management.
