The Family Health Team is a way of providing health care to you and your family. It is a team approach that brings together family doctors with other health care providers in order to coordinate the best possible care for patients. Everyone on the team works together to meet your needs. Their focus is on keeping you and your family healthy, not just treating you when you are sick.
When you enroll as a patient with a Family Health Organization, you are taking a step to ensure that you always have ready access to a team of health care providers.
Our medical office assistants (MOA's) work directly with the physicians to keep the office running smoothly and help you schedule appointments & relay important information from your physician. You may see them at the reception desk, talk to them on the phone or communicate with them through your physician.
Patient Flow Coordinators (PFC's) navigate the schedules of the Physicians, bringing you to your room, taking your blood pressure, weighing your baby and assisting the doctors with other duties in the pods.
