Covid Support Programs

The OSFHT Covid programs provide support during active covid infection and lingering health effects after covid infection. These programs are Nurse Practitioner led, and collaborate with other members of your health care team.

Covid@Home Monitoring Program

If you test positive for Covid-19, our nursing and nurse practitioner team will provide support and information to help you recover from your illness. Through phone calls, our clinical team will:

  • Provide advice on how to manage your symptoms
  • Review if you should temporarily stop any medications while you are sick
  • Review if you meet criteria for medications including Covid Antiviral medications
  • Provide advice on isolation and returning to regular activities

If you test positive for Covid-19, please call 519-470-3030 x103 and ask for reception to refer you to the Covid Program. You will generally get a phone call same day from our clinical team.

Post Covid Clinic

Do you think you have Long Covid?
Have you not fully recovered since being ill with Covid?
Has your health been worse since a Covid infection?

If yes, this clinic may be for you.  Appointments are in person, one on one to review your symptoms and their impact on your level of functioning after Covid illness. It uses an individualized approach to evaluate and manage symptoms of Long Covid. This clinic is led by a nurse practitioner who will collaborate with your doctor and may involve other members of our care team for support.

 

Please talk to your doctor or a member of your care team for a referral to this clinic.

Long Covid Information and Support Group

These monthly information and support sessions are for adults and their family members with possible or confirmed Long Covid. We will review Covid symptom management and support strategies and identify resources online and in the community. These sessions are offered monthly, with some online sessions and some in person sessions. Registration can be done for one session or multiple sessions.

This program is free but registration is required. To register, please call Program Reception at 519-470-3030 ext 103. Please indicate at the time of registration if you have a family physician part of the OSFHT or not, but this program is open to everyone.
