Chronic Health

The term chronic is often applied when the course of the disease lasts for more than three months.

Our programs are to help give our patients the resources and a better understanding to live a comfortable lifestyle.

Healthy Lifestyle Program
The next session starts on Tuesday, July 16 from 3:30-4:30 PM, and will run every week (and eventually every second week) at the same day/time
This program is for people who want to prevent or manage chronic disease, and want to make diet and lifestyle changes to help improve their health. You are eligible if you have diabetes or pre-diabetes, high blood pressure, high cholesterol, obesity, or increased risk for any chronic disease.

You will learn about:
– Healthy eating to prevent or manage chronic disease
– Slowly and safely increasing physical activity
– Problem solving, goal setting and staying motivated
– Stress and time management
– Taking charge of your health and wellness

This program is currently being offered in person at the Owen Sound Family Health Team. You do not have to be a patient of the OSFHT to join – the Program is open to everyone!

The program lasts for seven months, with one session every week (switching to every two weeks half way through, and then to once a month toward the end). Each session is one hour long.

This program is free, but registration is required. Please call Program Reception at 519-470-3030 ext 103 for more information or to register.
