On July 11, 2022, we first became aware that an unauthorized third-party had accessed our network and promptly responded by taking our systems offline and launching an investigation, including engaging cyber security experts to assist in these efforts.
We learned from our investigation that the incident may have resulted in the unauthorized access of your personal health information, for example, name, date of birth, contact information, health card information and health history (e.g., diagnosis, medication, referrals, visits, etc.).
There is no evidence that all of the information listed above was potentially accessed for each individual. It is out of an abundance of caution that we are notifying all patients whose personal health information was stored on our system at the time of the incident. We have also reported this incident to law enforcement and the office of the Information and Privacy Commissioner of Ontario (“IPC”).
Please know that protecting the personal health information of our patients is a top priority for us at Owen Sound Family Health Organization and Owen Sound Family Health Team. In addition to contacting our call center at the details listed below, patients have the right to file a complaint with the IPC.
